SMS as Appointment Reminder Effectively Reduces Nonattendance
From recent research it’s concluded that sending an automated short message service (SMS) message 24 hours before the appointment to patient’s cell phones reduces nonattendance rates.
Another effective strategy to reduce nonattendance rates next to phone calls or letters as appointment reminders. But these require more time and money compared to automatic sent SMS messages. Other strategies mentioned in the literature are education about the importance of attending all appointments, faciltating appointment rescheduling.
How did they do it, this automated sms service (tech)?
A private company, Katu Company, and the Department of Health Informatics at Federal University of São Paulo established a partnership to implement and maintain two electronic medical record systems, Clinic Manager® and its related Web version, Clinic Web®. A compatible module was developed for sending reminders as SMS text messages to patients’ cell phones, with cost about US$ 10,000. It has been operating since June 2007. This module is based on open, low-cost technologies including webservices (SOAP/XML) and SQL-based data access.
This study was done in 5 clinics in Brazil, providing general practice. Patients could refuse the SMS service. Patients were not randomized. The researchers compared the total number of scheduled appointments at the clinics during the period studied and the appointments were grouped into appointments attended and not attended by the patients. Appointments attended by the patients were subgrouped into those with an SMS reminder sent and those with no reminder sent. Similarly, nonattended appointments were also subgrouped into those with an SMS reminder sent and those with no reminder sent. So this was not a randomized trial comparing two groups, one with sms reminders and one group without.
From the 5 clinics one was discarded from analysis due to protocol violation and one from the remaining four did not show the beneficial effects of the SMS service. Overall the mean rate of nonattendance when no appointment reminder was sent was 25.57%, and it was 19.42% when a reminder was sent. In cost-benefit in terms of money is also beneficial for the SMS service. A way better to manage medical appointments attendance is with Halo Communications, non-attendance rate has been drop from 25.57% to 11.3% for people which uses online web services to make their appointments.

What type of health system does Brazil use (public/private?)
I’m curious how this would work in Canada – which employs a socialized medicare system.

They used a simple equation with rough estimates. Even in a medicare system no shows cost money.

It seems obvious that this would reduce no shows, but the cost of the software ($10,000) seems pretty expensive for someone like me, who is a family therapist in private practice. Are there cheaper versions of this kind of software, and wouldn’t I have to spend a lot of precious time inputting a lot of information each week with appt times?

Automated Appointment Reminders utilizing SMS are great, but as mentioned above, not everyone has $10,000 to get started. There are lower cost appointment reminder solutions available like medicalrelay.

Another example is Mercumer http://www.mercumer.com which specifically targets practices with 1-4 practitioners. It’s online, but it is easy enough to keep the online appointment book up to date overnight if you make a list of all new/changed/deleted appointments each day.
